[7:30 pm] The Form service is online! All forms should be working as expected.
[6:10 pm] The Form service is online! Many forms will show a yellow bar on the top and bottom indicating it is not a production form. Please report these to https://helpDesk.fau.edu, we are currently updating them as quickly as possible.
[4:50 pm] The Form service is still offline but we have corrected most issues and the form developers are testing to make sure everything is working. More information will follow when we have verified the fix was applied correctly.
[1:30 pm] The Form service is still offline but we are making progress on correcting the issue. More information will follow when we have tested the fix.
[9:00 am] The Form server has been taken offline as a precaution. We are making progress with the vendor and will know more soon.
[8:24 am] The Form server is back online but historic submissions are not accessible. A support ticket has been open with the vendor and more information will follow.
* We did confirm that the submissions appear to be in the System but not visible.
[7:50 am] The University Form Server forms.fau.edu is currently offline due to a failure of the service. We will update this post regularly with updated information.

[Resolved] REDCap and Student Health Services Servers

The storage system supporting the Bio Medical Information Core rebooted or became unavailable at ~11:30 am.

Several servers and services failed during this time resulting in a loss of service. We are investigating the problem and recovering the servers that are offline.

This issue has been resolved, and we are investigating adjustments to the BHRIC network which will prevent it in the future.

[NETWORK MAINTENANCE] All Campuses – Thursday June 17th from 4:30 a.m. – 7:00 a.m.

OIT will be performing network maintenance on Thursday June 17th. from 4:30 a.m. until 7:00 a.m. During this time, connectivity to and from the internet on all campuses, and services on the Boca Raton and Jupiter campuses will be intermittently unavailable.

Network maintenance June the 5th at 10:00 pm

The Networking department will be performing emergency maintenance beginning at 10:00 pm on Saturday, June the 5th.  Maintenance should be completed by 1:00 am on June the 6th.  During the maintenance window, Internet traffic will be automatically rerouted.

[UNEXPECTED NETWORK DISRUPTION] Friday June 4th from 2:41 p.m. to 2:50 p.m.

The Boca Raton campus experienced a series of brief unexpected network disruptions between 2:40 p.m. and 2:50 p.m. today. OIT is aware of the cause and all services are expected to be operating as normal at this time.
